New Patients

We are now accepting new patients. If you wish to become a new patient at Family Medicine at Lowry please print, fill out and submit your new patient paperwork, along with you medical records release form to our office via fax or regular mail. Once receive your new patient paperwork, and your medical records, we will contact you to schedule your appointment. Please note, that NO APPOINTMENTS will be scheduled without meeting the above requirements. Thank you in advance and we look forward to meeting you.

Is this an Emergency?

If you think a trip to an ER or Urgent Care is needed, you do not need our permission. You do, however, need to know which facilities are preferred by your insurance company. Only your insurance carrier has this information. It is important for you to know that before an emergency comes up. Look at our list of links to local hospitals and Urgent Care facilities.
