Q: I am sick or my child is sick, and there are no appointments
available. What can I do?
A: You do not need our permission to go to the Urgent Care or the Emergency Room. Please visit your local
facility as soon as possible, for immediate care. (Insert Hospitals/Urgent Care Link.)
Q: What if I cannot make my scheduled appointment?
A: Please contact our office at least 24 hours in advance to cancel your appointment, and avoid “No Show”
fees. If you have scheduled a same day appointment, please know that the “No Show” fee will apply,
should you not be able to attend your appointment.
Q: How do I get a refill of my medication?
A: Please contact your pharmacy. They will give us a call or fax.
PLEASE allow 3 business days for refill processing. (Insert the prescription refill link.)
Q: How do I know if I need a referral to see a specialist?
A: A good rule of thumb is…If you have to select a Primary Care Physician thru your insurance, you most
likely need a referral to see a specialist. If you have an HMO plan, you will most likely need a referral to see
a specialist.
Q: I want to talk to my doctor. How can I get a hold of
her?
A: Our doctor’s take pride in the time they allow for visits
with their patients. The best way to get in touch with your doctor, is to ask a question thru www.reachmydoctor.com,
contact our Office Manager & RN Denise Allen, or schedule an appointment to have a discussion with your doctor.
Please know that our doctors DO NOT check their voicemail on a regular basis. So, should you need to discuss something
with your doctor; please use one of the 3 available options.
Q: How do I request my Medical Records?
A: Please submit a Medical Records Request Form (add link for form.) Once we have received this form, please
allow 3 business days for your medical records to be released. Should you wish to purchase a copy of your
records for personal reasons, charges begin at $14, and go up from there. Please contact our office for more
information.
Q: When was my last physical/yearly exam?

Q: My doctor gave me samples, and I would like to get a
prescription for the medication. What do I do?

Q: What is the difference between a D.O. and an M.D.?
A: Both D.O.’s & M.D.’s complete four years of Medical
School. After Medical School, both M.D.’s & D.O’s
complete Residency Internship’s side by side. This could last
up to 7 years. Upon completion, they can choose to practice in any
specialty are of medicine. However, 65% of D.O.’s go into
Primary Care. Together, D.O’s & M.D.’s enhance our
American healthcare system. D.O.’s do bring something extra
to medicine. Osteopathic medical schools emphasize training their
students to be primary care physicians. They also practice the “whole
person” approach to medicine, instead of treating specific
symptoms or illnesses. In addition, D.O.’s focus on preventative
